To schedule a telemedicine visit with one of our providers, please follow these instructions:
1. Click here to schedule an appointment online
- If you have a preferred provider, select that one.
- If you do not have a preferred provider, or wish to schedule the next available appointment, select the provider ‘Request/Urgent Telemedicine Appointment’
2. Once selected, fill out the short form and schedule / request an appointment at your desired time
3. Once completed, you will receive a message via email and SMS to complete all registration and check-in processes from your device or computer
4. On the day of your appointment, you will receive a secure URL via email and SMS from your provider, to log into your appointment
5. Once logged on, share anything you’d like to share with your provider. They are there for you
6. At the end of your appointment, you will receive a feedback request. We would appreciate if you would fill it out, so we can continue to better improve our services
*Please note that you may be asked to allow audio/video services on your device from which your provider will be calling to conduct the telehealth visit*
